You fail a Mission when you don't meet the set conditions to win it or one of your survivors becomes incapacitated.

Injuries suffered will remain in place and your survivors will need to be patched up in the Hospital before you can send them on another Mission. Incapacitated survivors will suffer from severe injuries that require a lot of time to heal.
